I. Requirements for local suppliers

Please complete the Supplier Accreditation Application Form and upload all required documents directly through the online form.

Accreditation process
  1. Fill out the online application form.
  2. Upload the required documents per item.
  3. Submit the application for review.
  4. The SLDCC Procurement Team will review your submission and contact you within 48 hours for next steps.
BIR Form 2303 (Certificate of Registration)
Mayor’s Permit / Municipal License
SEC / DTI Registration
Business Permit
Detailed location sketch of the sales office and warehouse
List of products/items (brochures & catalogues) (if applicable)
Company profile (including list of clients)
